2.  Attach the fee to the form (you can view a list of fees list below).
3.  Have a current physical on file at Cathedral (athletes)
4.  Turn in your forms and fee in to the Cathedral Activities Office (S101)

  Athletes must have a current physical on file at Cathedral. Physicals are current for 3 years from the date of the exam. Most students have physical prior to 7th grade and prior to 10th grade. Book your appointments early – your athlete will not be able to participate without a current physical on file at Cathedral.
